Group Health Insurance – Blue Cross Blue Shield of Massachusetts

Option HMO Blue New England 1000, 1500, 2000 or Access Blue High Deductible

  • Must work an average of 30 hours or more per week
  • Eligible on 1st day of employment
  • Domestic partner rider available on most plans
  • Agency subsidizes the deductible through an HRA (Health Reimbursement Account) – Employee responsible for first portion of deductible and agency pays the remaining portion
  • Available pre-tax or post-tax

Group Dental Insurance – Blue Cross Blue Shield of Massachusetts

Option Low Plan or High Plan

  • Must work an average of 30 hours or more per week
  • Eligible on 1st day of employment
  • Employee pays 100% of the cost of an Individual or Family plan
  • May elect coverage separate from health insurance
  • Available pre-tax or post-tax

Vision Insurance – Eye Med

  • Must work an average of 30 hours or more per week
  • Eligible beginning the 1st of the month following employment start date
  • Employee + 1 and Employee + Family Options
  • May elect coverage separate from health insurance
  • Major discounts on frames and contact lenses

Short and Long Term Disability Insurance – The Hartford

  • Must work an average of 30 hours or more per week
  • Short and Long Term plans available
  • Short Term – Employee receives 60% of salary up to 9 weeks of disability
  • Long Term – Employee receives 60% of salary for full duration of disability
  • Tax free

Supplemental Life and AD&D Insurance – The Hartford

  • Must work an average of 30 hours or more per week
  • Purchased in increments of $10,000
  • The actual amount you can purchase may not be more than 3 times your annual earnings

Flexible Spending Account – Health Equity

  • Must work an average of 30 hours or more per week
  • Set aside monies to help defray expenses such as co-pays, deductibles, eye glasses and/or other health care-related expenses
  • Also may be used for Dependent Care Expenses such as day care or after school care
  • Pre-tax only and $2500.00 annual maximum; Roll-over available

Health Reimbursement Account – Health Equity

  • Health Equity keeps track of your Health Insurance deductible payments
  • Issues payment directly to YOU once your portion of the deductible is met so that you can pay the provider

Health Savings Account – Health Equity

  • Must be enrolled in the Access Blue High-Deductible Health Plan
  • You cannot be enrolled in Medicare in order to participate
  • If you have a family health plan, you cannot claim a dependent on another person’s tax return
  • Funds are owned by you, the participant and are pre-tax only
  • Set aside monies for the same expenses as the Flexible Spending Account without losing the funds at the end of the plan year. Funds will carry over from year to year and is portable if employment changes

403(b) Retirement Savings Plan – Security Benefit

  • Must work an average of 30 hours or more per week
  • Strictly voluntary – employee contributes 100% to the plan
  • Pre-tax only.
  • Employee may enroll upon hire and anytime thereafter

State Funded Tuition Remission – Providers’ Council for Caring Communities

  • Must work an average of 30 hours or more per week and have been employed at least 6 months
  • The Brien Center pays for the application fee
  • Applies only for Bachelor’s degree in Human Services
  • Applies only for employee’s who work in certain state contracts
  • Must meet all state requirements

Home and Auto Insurance – Liberty Mutual

  • Discounted Rates offered exclusively through The Brien Center on Home and Auto Insurance
  • Direct-billed or easy payroll deduction

Travel Time & Mileage

  • Paid travel time, plus 42¢ per mile for authorized agency business
  • Mileage is tax-free

Combined Leave Pool

  • Accrued in each of the 26 pay periods
  • New employees will accrue hours from their hire date
  • Leave Pool “Cash out” available (some limitations apply)

Bereavement Leave

All full-time and part-time employees are eligible for up to 5 consecutive days off with pay for the death of an immediate family member. The following relationships qualify for this leave time:

  • Spouse or domestic partner
  • Mother, father, sister, brother (including in-laws) · Children or grandchildren
  • Grandparents
  • Step family members
  • A person standing in loco parentis

Agency Holidays

All major holidays taken off will be paid from the employee’s earned leave pool. All agency programs, except those that are open 24-hours, 7 days, will be closed on the following major holidays:

  • New Years Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Christmas Day
